LEAVE OF ABSENCE
Q: Do I use leave of absence form if I am sick for a week?
A: No, this is not for day-to-day absences of less than ten (10) days.
 
Q: Do, I have to turn this form in to my principal or supervisor?
A: Yes, as this way you are communicating why you are requesting a leave.
 
Q: What types of leaves (request) require submission to the Assistant Superintendent – Human Resources for processing?
 
 
A: For more Classified information see section 6 of CSEA contract.
 
 
A: For more Certificated information see section 11 of MBUTA contract.
 
Q: What form do I need to turn in if I want to take a leave of absence for a pregnancy?
A: Complete a Request for Leave of Absence Form along with a doctor’s note and any other documentation. This form must be completed for any type of leave listed above.

 Q: What if I want to take off additional time after the initial leave is over? What forms do I need to complete?
A: You would additionally complete the Family Care Leave Form CFRA.
 
Q: What if I want additional time off when all my  paid leaves have run out?
A: To extend it even further would be an unpaid personal leave for family bonding. You would then complete a Request for Part Time Extended Unpaid Personal Leave of Absence Form
 
Q. If I need to take time off from work for an extended amount of time for a medical or personal reason.
A. Consult with your supervisor and forward to HR a completed ‘Request for Leave Form’ located at www.mbusd.org  under Departments> Human Resources> Key Documents tab.

Q: What if I get injured on the job?
A: Injured worker notifies supervisor. Supervisor/Injured worker immediately calls injury hotline
(Company Nurse) at 1-888-770-0929. You will also need to complete a DWC-1Form. See Injury Reporting for more detailed information.

Q. I am applying for a loan and the agency needs a Verification of Employment from MBUSD.
A. Advise the agency to call Human Resources at 310 318 7345 X5951. We will handle it from there.
 
Q. I have questions about my rights as an CSEA contracted employee . Questions about Evaluations, Compensation, Lay Offs, Transfers, Benefits etc.?
A. Refer to your CSEA Contract and Merit System located at www.mbusd.org under Department> Personnel Commission> CSEA Bargaining Agreement and Merit System tabs.
 
Q. Where is a copy of my job description?
A. Go to www.mbusd.org under Departments> Human Resources> Job Descriptions tab.

Q. I would like to resign or retire, what is the process?
A. Complete and sign a Retirement/Resignation form, have your supervisor sign and forward it to HR.
 
Q. How do I apply for another position at MBUSD? Do I need to fill out an application?
A. Go to Edjoin, register for an account and apply.  We have gone paperless and no longer accept paper applications.
