Welcome to the Health and Welfare Benefits page. This page provides information about the Health and Welfare benefits offered to employees.
 
The benefits plan year runs from October 1st through September 30th of each year.
 
  2023 - 2024 ENROLLMENT & PLAN INFORMATION
 
 
2023-2024 OPEN ENROLLMENT IS CLOSED
(OE was from 8/9/23 - 9/12/23)
 
   Open Enrollment started on August 9, 2023.  Elections made during Open
   Enrollment went into effect on October 1, 2023. Making changes to benefit elections
   after the Open Enrollment window may only be done if you are a new employee or
   if you experience a qualifying event (e.g., marriage, divorce, birth of a child, etc.)
 
   Please review the 2023-24 Enrollment Guide to learn more about the
   plans. This is a "flipbook" guide that is best viewed fullscreen.
   A PDF of the guide may also be downloaded here. 
 
   Benefit elections may be made through the Ease Benefits portal
   during Open Enrollment, or within 30 days of a Qualifying Event.
   
    All 50%+ employees are eligible to participate in the health benefit plans.
 
    All employees should log on to www.MBUSD.Ease.com to designate beneficiaries
    for the District-provided Life insurance.

If you selected any of the Anthem Blue Cross plans, we encourage you to go to Anthem's website, and register. You can register using either the ID number from your insurance card, or your social security number.

  American Fidelity- Supplemental Insurance
To see information on available supplemental insurance options (e.g., a Flex Spending Account (FSA) or Dependent Care Spending Account (DCSA)) through American Fidelity (AF), go to https://enroll.americanfidelity.com/377E2D79. AF Open Enrollment is from 8/21/23 to 9/12/23.  To schedule a virtual appointment. Select a date and time, and an AF representative will contact you, send a link, and walk you through enrollment and answer any questions.

  Tax Sheltered Annuity- (403b/457)
  Medicare Advantage Plans for Employees or Retirees with Medicare:
Retirees and employees over 65 who enroll in Medicare for medical coverage now have the option to enroll in District-provided "Medicare Advantage Plans" through Kaiser or SCAN. These plans provided enhanced coverages and benefits to the basic Medicare coverages. Information on each plan can be found here:

  The Hartford - District provided insurance:
The district pays for long-term insurance and life insurance coverage for all employees that work at least fifty percent at no cost to employees. Click the appropriate link below for an overview of these benefits:
  Anthem Blue Cross Optional Travel Coverages 
Traveling during your summer break?  Anthem Blue Cross has provided the following member information for those who may be traveling internationally, or who may need to seek out-of-area non-emergency and urgent care services:
  COBRA
COBRA is the Consolidated Omnibus Budget Reconciliation Act which amended the Employee Retirement Income Security Act, the Internal Revenue Code, and the Public Health Service Act to provide continuation of group health coverage that otherwise might be terminated. COBRA provides certain former employees, retirees, spouses, former spouses, and dependent children the right to temporary continuation of health coverage at group rates.
