MBUSD is now using an online form to collect requests and approve field trips. Please use the appropriate form below to submit:
Prior to submitting, please review the Board policies and
field trip procedure documents linked below to ensure you are submitting trip requests in a timely manner as some require prior Board approval and/or lengthy lead times.

PARTICIPATION AGREEMENTS & WAIVERS:
Forms to print, distribute and collect for Student, Parent, and Chaperone Participation Agreements/Waivers are linked below.
Volunteers/chaperones must be cleared through LiveScan (fingerprinting). Information on scheduling and the forms needed is found here:
