COVID-19: Work Status, Reporting to Work, and Returning to Work


Thank you for all you do to make our MBUSD world go ‘round so that students can continue to learn in person. We must do our part to stay healthy by keeping a six-foot distance as much as possible, wearing an upgraded mask (e.g., surgical, N95, KN95), washing hands frequently for at least 20 seconds, and staying at home when we are not feeling well.

Therefore, it is always important to check LACDPH isolation and quarantine guidelines. The LACDPH Decisions Pathway is also a useful resource. 
If you have tested positive for COVID-19:
  • Use this MBUSD Staff COVID Reporting Form link to submit information about your positive case. Dawnalyn Murakawa-Leopard, Deputy Superintendent, will receive responses and contact you via email with the next steps. (LACDPH isolation guidelines.)


  • You may use an FDA-approved over-the-counter antigen self-test and, if you do so, must submit your results to this link.
If your vaccination status has changed - and especially if you have received a third/booster dose and are considered up to date with COVID-19 vaccines - please submit documentation of your vaccination and booster records to
Quarantine/Isolation Pay for employees


While the Federal Supplemental Paid Sick Leave (SPSL) bill expired as of September 30, 2021, MBUSD is providing paid quarantine/isolation leave to employees who have had to isolate or quarantine due to a COVID-19 exposure.

You may go back to October 1, 2021, to submit updated Certificates of Absence retroactively, but you must provide exact dates. Documentation may be requested on an as-needed basis.

Steps to receive quarantine/isolation pay:

1) Go to InformedK12 to access the Certificate of Absence Form.
2) Fill out the top of the form will all the required information (boxes in red).
3) Select “Q” “Quarantine.”
4) List the dates of the days that work was missed due to a COVID-19 quarantine or isolation requirement.
5) Send to the payroll specialist who oversees your department/site.
6) Also, when submitting to the payroll specialist, copy the appropriate HR Technician: for classified for certificated for substitutes
